Not really. You can be attracted to men and not identify as gay. Regarding the little one... Even if he has a vagina now, Tachibana still fundamentally identifies as a man and prefers he/him pronouns so he's not a woman.

Gay is a word originally meaning happy. You can see examples of this in old books like Nancy Drew and other forms of media from the same time. At a later point the word gay became a slur, so people sometimes don't like using that label to identify themselves. Nowadays a lot of people like to call themselves Men Loving Men (MLM) or Women Loving Women (WLW). Sure, people still use terms like Lesbian, Gay, and Bisexual, but not everyone choses to use those specific labels. In this series, Tachibana and his friend explicitly say that they don't identify as gay. Perhaps not using that specific wording, but I believe that's what they meant. Though it doesn't help that the LGBTQ+ community in Japan is pretty heavily repressed. It could be that they don't or didn't have access to the language that is used to describe what they're feeling because of this, so they don't know how else to put it.
